Good care is measurable. For anyone at risk of pressure sores, staff should complete a risk assessment, write a clear plan, put the plan into action, and document what happened. When those steps are missing, early damage is often missed too.
“Duty of care” means doing what a reasonably competent provider would do. In practice: reposition regularly, check skin daily, keep hydration/nutrition on track, and use pressure-relieving equipment as needed. Standards give teams a checklist; records prove it happened.
